Assessment Process

  • To ensure we’re the right service for you, complete our screening questionnaire online or over the phone with our admin team prior to booking.

  • Once your initial assessment is booked, our ADHD assessment coordinator will send you a number of tests and questionnaires to complete prior to your meeting with the Nurse Practitioner (NP). Please complete these as soon as possible (at least 48 hours in advance of your appointment) so we have time to review them.

    If you have any questions, you can contact the assessment coordinator for help.

  • You’ll meet with one of our NP's for a 30 minute intake appointment. During this interview you’ll be asked about your symptoms and your goals for treatment.

    Based on the results of your intake and testing your NP will help you decide whether it makes sense to proceed with a full ADHD assessment or whether another service might be a better fit.

  • Following your intake you will meet with the NP for a 60 minute assessment appointment. At this meeting the NP will ask you more details about your symptoms and how they have impacted you throughout your life.

    If you have them, it can be helpful to bring your school report cards to this meeting or ask your parents for more details about your childhood.

  • Following your assessment, the nurse practitioner will provide you with a report of their findings, including their diagnosis, when applicable. If you would like this information shared with your family doctor, we can do this too.

    In some rare cases, to achieve a clear diagnosis, a referral for testing or assessment by another provider may be required.

  • At the end of your interview, the nurse practitioner will discuss their treatment recommendations with you. This could include medication, lifestyle strategies, or ADHD coaching/counselling. In many cases it will involve a combination of all three. Medical treatment may be offered within Blueprint, however, in some cases treatment will be referred to your family doctor. The Nurse Practitioner will refer you to our ADHD coaches or psychotherapists for the non-medical aspects of your care.

  • The total cost for ADHD assessments is 600$ (160$ for the intake and 440$ for the assessment)

    Follow up treatment appointments are 85$

  • Adults 17+ located in Ontario at the time of the assessment.

  • No, but we do have a referral form to collect information from your counsellor or physician. This will aid the nurse practitioner in their assessment, but it’s not required.

  • No, but in order to curate the most effective treatment plan, an assessment is strongly recommended. While counselling and medical treatments for ADHD are both effective on their own, the best outcomes generally come from a combination of both that is informed by a comprehensive assessment.

  • We can help you find the right treatment. But we want to get to know you first. All new patients start with an intake as it’s important that our nurse practitioners have enough time to get to know you and your personal medical history before prescribing any medication.

    If you have a recent assessment, completed by a provider with specialized training, you can contact our assessment coordinator to discuss your situation and arrange sending of reports.

  • Unfortunately not, nurse practitioners in Ontario are not able to bill OHIP directly. ADHD assessments for adults are difficult to obtain within the public health care system as there are few physicians specializing in this area and they often have long waitlists. Our assessments are comprehensive but also more affordable than a private psychological evaluation.

    The assessment fee may be eligible to be claimed as a medical expense on your income taxes and some extended health benefits (particularly health care spending accounts) may cover the cost of the assessment.

  • Diagnosing ADHD and mental health concerns is a complex process and we want to make sure we’re getting it right. Our Nurse Practitioners will work with you at every step along the way to ensure you receive a comprehensive assessment and provide you with a diagnosis if you meet the criteria. In some cases further assessment may be required if there may be other diagnoses causing your symptoms. If this is the case the nurse practitioner may need to meet with you a couple more times before providing a diagnosis or they may refer you to another specialist for further testing and assessment.

  • Yes, nurse practitioners are independent medical providers that are authorized to diagnose, order diagnostic tests and prescribe medication. Our nurse practitioners have a specialized practice in ADHD and mental health care, and they offer a comprehensive assessment that goes beyond what is typically offered in the provincial health care system.

  • Yes, if you fit the criteria for medical treatment our nurse practitioner can initiate treatment or can send treatment recommendations to your family doctor. Keep in mind that most medications for ADHD are controlled substances and a prescription would be at the discretion of your physician. Given the specialized expertise of our Nurse Practitioners, they are comfortable helping you find the right medication and the right dosing prior to transferring you to your family doctor for maintenance.

  • It depends. Our nurse practitioners can complete forms for school or workplace accommodations, but some tax and insurance forms need to be completed by your primary care provider who knows you best. If you’re unsure, or you don’t have a primary care provider, please check with our assessment coordinator.
